We are looking for a Registered Nurse to join our home health team. The ideal candidate will have a passion for providing quality care to patients in a home health setting, and be able to demonstrate excellent communication and organizational skills. The successful candidate will provide assessments, develop care plans, administer medications, and monitor patient progress. Responsibilities: Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by physicians. Conduct initial assessments of patients and families to determine home care needs. Perform thorough physical assessments and gather comprehensive medical histories. Develop and revise individualized plans of care based on patient status and evolving needs. Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient. Responsively address inquiries and concerns from patients, caregivers, and medical teams. Provide counseling and support to patients and caregivers before, during, and after treatment. Supervise home health aides as required. Identify discharge planning needs and proactively implement them as part of the care plan development.

Homecare Hospice Registered Nurse

We are looking for a Registered Nurse to provide care for people who are terminally ill. The ideal candidate will have a passion for providing quality care to patients in a home health setting, and be able to demonstrate excellent communication and organizational skills. The successful candidate will be responsible for providing assessments, developing care plans, administering medications, and monitoring patient progress.
Responsibilities:
- Frequently visiting patients to monitor vital signs and physical comfort levels.
- Make sure the clients' medications are refilled.
- Informing physicians about changes in patients' conditions.
- Notifying attending physicians, hospice staff members, and family members of patients' deaths when they occur.
- Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
- Preparing and adjusting patient care plans as needed.

Homecare Physical Therapist
Full-timeQualifications:
Education and Licensure:
- Possess a degree from an accredited program in Physical Therapy.
- Hold a valid license to practice as a Physical Therapist in the State of Massachusetts.
Experience and Skills:
- A minimum of one (1) year of homecare experience as a physical therapist, (working in a hospital does not count as homecare and private duty does not count as homecare experience).
- Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
- Maintain current BLS CPR certification.

- Has working knowledge in a home care setting.
- Valid U.S. Driver's License.

Responsibilities:
Patient Care:
- Deliver physical therapy services in accordance with a physician's plan of care and the state Physical Therapy Practice Act.
- Assess and evaluate therapeutic, rehabilitative, and functional status, including home environment assessment.
- Perform assessments for muscle strength, mobility, gait, and range of motion, identifying potential for rehabilitation.
- Administer physical therapy treatments and provide instruction on the use and care of therapeutic appliances.
- Report the patient's reaction to treatment or changes in condition to the physician.
Program Management and Collaboration:
- Initiate and manage physical therapy programs, collaborating with other healthcare professionals and family/caregiver members involved in the patient's care.
- Instruct other personnel, family members, and caregivers on specific phases of physical therapy and the goals of the treatment plan.
- Offer training to patients on the use of prosthetic devices.
- Identify additional home health services needed by patients and refer as necessary.
- Contribute to discharge planning and participate in case conferences.
Documentation and Education:
- Prepare and submit clinical and progress summaries based on the achievement of goals.
- Provide physical therapy consultation to home families/caregivers as required.
- Conduct in-service education programs for nursing organization personnel as needed.
- Engage in peer consultation process.
- Supervise physical therapy assistants in compliance with organization policy and state regulations.
- When therapy is the sole skilled service, instruct, supervise, and evaluate home health aide care on a bi-weekly basis.

The successful candidate will be responsible for providing nursing care to patients, assessing their needs, developing individualized care plans, and monitoring their progress.
Responsibilities:
- Basic wound care including cleaning and bandaging injured areas.
- Recording the patient's history, updates and condition.
- Giving medication as prescribed by the physician.
- Performing daily care tasks such as blood pressure, temperature, and weight.
- To effectively assign, organize, and oversee all facets of nursing care for patients, their families, and the broader community, while maintaining timeliness and cost-efficiency.
