Home Care Hospice Registered Nurse (RN)

Responsibilities:

● Frequently visiting patients to monitor vital signs and physical comfort levels.
● Make sure the client’s medication is refilled.
● Informing physicians about changes in patients’ conditions.
● Notifying attending physicians, hospice staff members, and family members of patients’ deaths
when they occur.
● Collaborating on terminally ill patients’ care with physicians, social workers, chaplains, nursing
assistants, and other stakeholders.
● Preparing and adjusting patient care plans as needed.

Requirements:

● Has knowledge and skills in dealing with death and dying.
● A minimum of 1 – 2 years experience as a hospice nurse in home care settings
● License RN in {location}
● BLS
● US Driver’s license
● Experience in Homecare Homebase is a plus
