We are seeking a Licensed Practical Nurse (LPN) to join our home health and outpatient team. The
ideal candidate is compassionate, detail-oriented, and committed to delivering high-quality care in a
home health setting. Responsibilities include providing direct patient care, assisting with care plan
implementation, administering medications, monitoring patient progress, and collaborating with the
healthcare team to ensure optimal patient outcomes. Strong communication and organizational skills are
essential.

Responsibilities:
● Conduct comprehensive assessments of patients and develop individualized care plans.
● Administer medications and treatments as prescribed by physicians.
● Monitor vital signs and assess the patient’s condition during each visit.
● Collaborate with a multidisciplinary team to provide holistic care.
● Educate patients and their families on managing their conditions and promoting health.
● Maintain accurate and up-to-date patient records.

Requirements:
● 1 year of recent Home care experience (private duty does not qualify)
● Valid LPN license in {location}
● BLS
● Driver’s license
● Experience in Homecare Homebase is a plus
